 COVID-19 Screening Tool

Dear Families,

This is a friendly reminder to please ensure that you have completed the necessary COVID-19 screening survey prior to your child attending school. The survey only needs to be completed on the days that your child is physically in school. The link to the screening can be found on the main page of the district website and is also listed below for your convenience. Please remember that the survey is required as per New York State guidelines. Please also see the flyer attached for directions on how to download the QR code.
